Complete Damaged Merchandise Claim Form

All required fields must be completed with accurate information
Retain a copy of the claim form for your records.
A customer service representative will respond via telephone or email.
Please provide a telephone number were you can be reached Monday – Friday, 9AM - 5PM ET
Email omitting or providing erroneous contact  information will be discarded.
Generalsuperstore prides itself on customer service and customer satisfaction.  We strive to make your shopping experience
simple and problem free.  Unfortunately, circumstances do arise where merchandise is lost or damaged during shipment.   
Because we rely on third party shippers to deliver your merchandise to you, we ask that you read and follow the instructions
posted below regarding damaged merchandise.

1.  ALWAYS inspect your package(s) or shipment immediately upon receipt. Inspecting your package or
shipment also means that you must open your box(es) and inspect the merchandise inside!    

2.
 If your merchandise is damaged, YOU MUST COMPLETE THE FORM BELOW WITHIN TWENTY-FOUR (24) HOURS
OF RECEIPT TO NOTIFY OUR OFFICE OF THE PROBLEM.
Refrain from using the product and repackage in its original
carton. Remember, It is imperative you retain all packaging material (bubble wrap, styrofoam, peanuts, etc…) as it is a very
important part of the inspection process.  

3.  If the merchandise is visibly damaged upon arrival; refuse shipment and have driver return package marked damaged.
Immediately notify our office of refusal of delivery via form below.

4. Please complete the Damaged Merchandise Claim Form below to advise us that your merchandise arrived damaged.  
Remember, your claim form must be received by us within 24-hours of receipt of the damaged merchandise. The following
information will also be required:


5.  All merchandise damage claims received within the required 24-hour time frame will be assigned a Damaged Merchandise
Claim Number.   This claim number is not the claim number of the shipper, but is an internal claim number provided by
Generalsupestore to aid us in assisting our customers.

Claims for damaged merchandise will be rejected if…
...the packing material (bubble wrap, styrofoam, peanuts, etc…) and shipping container/box are missing or discarded
by customer.
...claim form is not submitted within 24-hours of delivery.
...merchandise is returned without an approval number.
...supporting documents such as pictures of damages are not provided with claim.
...the merchandise is not retrieved by FedEx within 3 attempts. Time is of the essence, therefore all damage claim(s)
must be completed within 5 business days from reporting the damage(s).
